What are the steps to purchase?

1) Fill out the form and you will receive an email with pricing and a link to purchase.

Can I share this package with my family or spouse?

Yes. This limited time deal offers the ability to share all rehab packages with family members.

Do the visits purchased expire?

Yes. They expire in 12 months from purchase date. However, our general packages expire in 3-6 months, so the 12 months expiration date is part of the limited time offer.

Can I use my HSA or FSA dollars to purchase the enrollment package?

Yes. You can use both your HSA and FSA money towards the purchase of this package.

Do you bill insurance?

Faith in Motion is an out of network clinic meaning we do not directly work with insurance companies. However, we do provide itemized bills following visits that can be turned into your insurance company by you for reimbursement depending on your coverage.
